IBX Personal Choice 65 PPO 2027: Plans, Costs and Benefits

Reviewed October 17, 2026. IBX Personal Choice 65 is Independence Blue Cross’s Medicare Advantage PPO for southeastern Pennsylvania. For 2027, IBX lists three individual Personal Choice 65 PPO plans: Personal Choice 65 Rx PPO, Personal Choice 65 Achieve Rx PPO and Personal Choice 65 Medical-Only PPO. The Rx PPO costs $207 a month in Chester, Delaware and Montgomery counties and $242.50 in Philadelphia and Bucks counties in 2027, with a $5,950 in-network out-of-pocket maximum.

Quick answer: Personal Choice 65 PPO lets you see in-network and out-of-network doctors without referrals. In 2027 the Rx PPO has $0 primary care visits, $40 specialist visits, a $0 drug deductible, a $1,500 dental allowance and a $30 quarterly OTC allowance. IBX Personal Choice 65 PPO plans for 2027

Figures below come from IBX’s 2027 Summary of Benefits and Annual Notices of Change, all dated October 1, 2026. (IBX plan documents) The plans are offered by QCC Insurance Company, an Independence Blue Cross company.

IBX’s 2027 plan documents don’t include a plan named “Personal Choice 65 Prime Rx PPO.” If you had a Personal Choice 65 plan in 2026, your Annual Notice of Change (mailed by September 30) tells you which 2027 plan you’ll be in and what’s changing.

2026 vs 2027: what changed

Other Rx PPO changes for 2027: dental implants are no longer covered, the Dementia Support Program ended, Tier 5 drugs are limited to a one-month supply, and Dexcom continuous glucose monitors were added. On the Achieve Rx PPO, the out-of-pocket maximum rises from $6,750 to $9,850, the drug deductible from $375 to $700, specialist visits from $55 to $60, and the OTC allowance and fitness benefit were dropped.

Extra benefits in 2027

Prescription drug coverage

The 2027 Rx PPO has a $0 drug deductible. Tier 1 drugs cost $9 and Tier 2 $20 at standard pharmacies ($0 at preferred pharmacies), Tier 3 is 25%, Tier 4 26% and Tier 5 33%, and covered insulin is $35 a month. Medicare’s Part D out-of-pocket cap is $2,400 in 2027, after which covered drugs cost $0. Where Personal Choice 65 is available and how the network works

The Rx and Achieve Rx PPOs serve Bucks, Chester, Delaware, Montgomery and Philadelphia counties. As a PPO, you can see out-of-network providers who accept Medicare, but you pay more and the higher combined MOOP applies. How to enroll

1. Confirm you have Medicare Parts A and B and live in the service area.
2. Check your doctors and drugs in IBX’s 2027 provider directory and formulary.
3. Enroll during Open Enrollment (October 15 to December 7, 2026) for coverage starting January 1, 2027, or during your Initial Enrollment Period or a Special Enrollment Period.
