Molina Healthy Food Card 2027: Amounts by Plan, Stores and Balance
Reviewed October 25, 2026. The Molina healthy food card isn’t a separate card: on Molina’s Medicare dual-eligible plans, food and produce money is loaded onto the same prepaid debit card as the OTC allowance, which Molina’s 2026 and 2027 plan documents call the MyChoice card. You can only use the food part if you meet your plan’s chronic-condition rules, and the amount depends on your plan and county.
Quick answer: For 2027, the Molina D-SNP plans we checked load a combined $200 a month in Iowa (H4367-001) and $184 a month in Wisconsin (H5209-006) for OTC items, food and produce and utilities, and Molina’s Sacramento County, California plan (H3038-004-004) has $25 a month for OTC plus a $45 monthly allowance that includes food and produce. Money is loaded monthly and doesn’t roll over. Molina’s Medicaid plans don’t offer a Medicare-style food card, though some states have meal programs or rewards cards.

How the Molina healthy food benefit works
Molina’s 2026 Summaries of Benefits describe the MyChoice card as a “pre-funded debit card.” On Molina Medicare Complete Care and Complete Care Plus plans, which are Dual Eligible Special Needs Plans (D-SNPs) for people with both Medicare and Medicaid, the card can carry:
- An OTC allowance for over-the-counter health items, available to all members of the plan
- A food and produce allowance, available only to members who qualify for Special Supplemental Benefits for the Chronically Ill (SSBCI)
- On some plans, money for utilities or transportation for SSBCI-eligible members
Funds load monthly. Molina’s 2026 summaries say unused money doesn’t carry over to the next month or plan year, so plan to use it each month.

Who qualifies for the food allowance
Food benefits on Medicare Advantage plans are usually SSBCI benefits, which Medicare rules limit to members with a qualifying chronic condition who meet the plan’s other criteria. Molina’s summaries point to Chapter 4 of each plan’s Evidence of Coverage for the list of conditions. In general you need:
- To be enrolled in a Molina plan that offers the food benefit
- A qualifying chronic condition (the list is in your Evidence of Coverage; diabetes and heart disease are common examples)
- To complete any step your plan requires, such as a Health Risk Assessment or a provider confirming your condition
If you’re not sure whether you qualify, call the Member Services number for your plan (below) and ask whether the food allowance is active on your card.
What food you can buy
Molina’s plan documents describe the benefit as “food and produce.” On Medicare plans, these allowances typically cover healthy groceries such as fruits, vegetables, meat, fish, dairy, bread and pantry staples. They don’t cover alcohol, tobacco or prepared hot foods. The card checks eligibility at the register, so items that don’t qualify will be declined. Molina hasn’t published a 2026 itemized food list on its public site, so check the member portal or your card materials for the exact list.
Where to use it and how to check your balance
You use the card at participating stores. Molina’s flex benefits site, Flex.MolinaHealthcare.com, is where members check balances and find stores. Molina’s 2023 MyChoice card FAQ for agents also lists:
- Balance by phone: (800) 665-0898 (TTY 711)
- Card activation: (866) 898-9795
- OTC orders online at Molina.NationsOTC.com
Those numbers come from a 2023 document, so if one doesn’t work, use the Member Services number on the back of your Molina ID card. See our guide to checking an OTC card balance for general tips, and Molina OTC benefits for the OTC side of the card.
Molina Medicare in 2027
Molina confirmed in its third-quarter 2026 results that it is exiting its regular Medicare Advantage prescription drug (MAPD) product for 2027 to focus on its dual-eligible business, so non-dual plans such as Molina Medicare Choice Care shouldn’t be offered in 2027. Its D-SNP plans continue, with 2027 allowances like those in the table above.
If you’re a Molina Medicare member, read your 2027 Annual Notice of Change. If your plan is ending, you can choose a new plan during Open Enrollment (October 15 to December 7, 2026). Our annual plan review checklist can help.
Molina Medicaid food and meal programs
Molina’s Medicaid plans don’t use the MyChoice card, but some states offer food help:
- Iowa: the Healthy Rewards Program card can be used for SNAP-eligible food, OTC items and gas, using rewards earned for screenings and other healthy activities. Molina’s Iowa page also lists $30 worth of OTC items every three months.
- Michigan: Molina lists medically tailored home-delivered meals, healthy home-delivered meals, a healthy food pack and a produce prescription program; details are in the member handbook.
- Washington (Apple Health): medically tailored meals from Mom’s Meals, up to 28 meals after an inpatient stay.
- California (Medi-Cal): medically tailored meals and medically supportive food through Community Supports, with a provider referral.
