Zing Health 2027: Plans, Benefits and What Changed
Reviewed October 10, 2026. Zing Health’s 2027 Medicare Advantage plans cover select counties in Illinois, Indiana, Michigan, Mississippi, Ohio and Tennessee. The lineup includes HMO and PPO plans, chronic condition special needs plans (C-SNPs) for diabetes and heart conditions or dialysis, and “Elite” plans built around specific provider groups. The two Illinois C-SNPs we checked keep a $0 premium and a $75 quarterly OTC allowance, but their out-of-pocket maximum rises to $6,900 and the dental maximum falls to $2,000.
Quick answer: If you have Zing Health now, read your 2027 Annual Notice of Change before Open Enrollment (October 15 to December 7, 2026). Several plans have new names for 2027, and some benefits went up while others went down. Member Services is 1-866-946-4458 (TTY 711).
What is Zing Health?
Zing Health describes itself as “a Medicare Advantage HMO plan with a Medicare contract.” It was founded in 2019 by physicians Eric Whitaker, MD and Kenneth Alleyne, MD, with co-founders Health2047 and healthcare executive Garfield Collins, and started enrolling members in Cook County, Illinois in 2020. Its 2027 provider directories describe Zing Health of Michigan, Inc. as an HMO, HMO C-SNP, PPO and PPO C-SNP with a Medicare contract. Plans fall under two contracts: H4624 (HMO) and H6876 (PPO).

2026 vs. 2027: what changed on the Illinois C-SNPs
Zing Health’s 2026 and 2027 Summaries of Benefits for its two Illinois diabetes and heart plans show these changes. H4624-010 was called Zing Essential Wellness Diabetes & Heart IL in 2026 and is Zing Select Diabetes & Heart IL in 2027.

Other 2027 details from the same summary: the H4624-010 plan lists a $0 primary care copay, a $20 specialist copay ($0 for cardiology, endocrinology and some others), a $400 Part D deductible and a $0 health club membership. H4624-028 lists a $10 specialist copay and a $350 Part D deductible. OTC money doesn’t roll over. To get food and utility money you need a qualifying chronic condition, a high risk of hospitalization and a need for intensive care coordination, and for 2027, 2 months of continuous enrollment.
We couldn’t find 2027 Summaries of Benefits for Zing’s Indiana, Ohio, Tennessee, Mississippi or Michigan plans on its public site, so check those plans’ documents directly.
The Zing Health card in 2027
Zing Health’s 2027 catalogs call it the “Zing Health card.” Zing uses two benefit vendors, and your welcome materials tell you which one your plan uses:

Zing Health star rating
Zing Health says its H4624 contract had 3.5 stars for 2025 and 2.5 stars for 2026, and it attributes the drop to the Change Healthcare cyberattack. CMS released the 2027 Star Ratings on October 8, 2026. Check Medicare Plan Finder at Medicare.gov for Zing’s 2027 rating before you enroll; star ratings affect plan quality comparisons and, for 5-star plans, special enrollment options.
Should you choose Zing Health for 2027?
Zing’s C-SNPs can make sense if you have diabetes, a heart condition or kidney failure on dialysis and your doctors are in Zing’s network. Before you choose, compare:
- The out-of-pocket maximum, which rose sharply on the Illinois plans
- Whether your doctors, hospitals and dialysis center are in network (Elite plans use a narrower group of providers)
- Your drugs, the Part D deductible and pharmacy network
- OTC, food, dental, vision and transportation benefits you’ll actually use
- The plan’s 2027 star rating
