As of this review, Humana lists Humana Healthy Horizons Medicaid plans in Florida, Illinois, Indiana, Kentucky, Louisiana, Ohio, Oklahoma, South Carolina and Virginia, plus integrated dual special needs plans for people with both Medicare and Medicaid in Illinois, Indiana, Michigan, South Carolina and Virginia. Members get their state’s Medicaid benefits plus extras such as Go365 for Humana Healthy Horizons, a rewards program for healthy activities.

Quick answer: You can’t buy Humana Healthy Horizons directly. Apply for Medicaid in your state, then choose Humana during plan selection if it’s offered where you live. Current members can manage their account at account.humana.com (MyHumana) and should use the Member Services number on their ID card.

Humana Healthy Horizons plans cover the Medicaid benefits each state requires, including medical, behavioral health and pharmacy coverage. Go365 for Humana Healthy Horizons

Go365 is a rewards program for Humana Healthy Horizons Medicaid members in Florida, Indiana, Kentucky, Louisiana, Ohio, Oklahoma, South Carolina and Virginia (Humana):

• Members 18 and older register in the Go365 app with their Medicaid member ID.
• You earn rewards for activities such as a wellness visit with your doctor or working toward health goals like quitting smoking.
• Rewards can be redeemed for gift cards in the Go365 Mall.
• Rewards have no cash value and must be earned and redeemed in the same program year.

Humana Healthy Horizons for people with Medicare and Medicaid

In Illinois, Indiana, Michigan, South Carolina and Virginia, Humana offers integrated dual special needs plans (FIDE or HIDE D-SNPs) that combine Medicare Advantage with Medicaid benefits in one plan.
