Humana Vision Plans 2027: Eye Exams, Glasses Allowance and Providers
Reviewed October 15, 2026. Most Humana Medicare Advantage plans include routine vision care: a $0 routine eye exam once a year at an in-network provider and a yearly allowance toward glasses or contact lenses. The allowance depends on the plan. In the 2027 plans we checked it ranges from $150 to $350 a year, and some plans pay more at “PLUS” providers in the Humana Medicare Insight Network. Humana also sells separate vision insurance, Humana Vision PLUS, for people who want vision coverage outside a Medicare Advantage plan.
Quick answer: To find who accepts Humana vision near you, use Humana’s provider finder at Humana.com/FindCare (or the “find an eye doctor” page), choose your plan’s network and search for optometrists. Then tell the office you’re part of the Humana Medicare Insight Network. Before you pick a 2027 plan during Open Enrollment (October 15 to December 7, 2026), compare the vision allowance in each plan’s 2027 Summary of Benefits.

How Humana vision works in Medicare Advantage
Routine vision is a supplemental benefit, meaning it’s an extra that Humana adds on top of what Original Medicare covers. Humana’s 2027 Summaries of Benefits describe it like this:
- Routine eye exam: $0 copay at an in-network provider, up to one exam per year.
- Eyewear allowance: a yearly “maximum benefit coverage amount” for contact lenses, or eyeglass lenses and frames.
- PLUS providers: some plans pay a higher allowance at providers marked with the PLUS indicator in the Humana Medicare Insight Network.
- Calendar year: the allowance resets each year and unused amounts don’t carry over. The standard and PLUS amounts can’t be combined.
- Out-of-pocket maximum: what you pay for supplemental routine vision doesn’t count toward the plan’s medical out-of-pocket limit.
Some plans give you a choice instead of a higher allowance. Humana’s Giveback HMO plans in Florida, for example, offer either the eyewear allowance or two pairs of select eyeglasses a year at no cost from a network optical provider.
Humana vision 2027: plan examples
Humana’s October 1, 2026 announcement says dental, vision and hearing benefits are “available across plan offerings” for 2027, and its plans cover about 2,600 counties in 45 states and Washington, D.C. (Humana, October 1, 2026) Allowances vary by plan, as these 2027 examples show:
| Plan (2027) | Routine eye exam | Eyewear allowance per year |
|---|---|---|
| Humana Gold Plus H1036-335 (HMO-POS), Brunswick, Davidson, Davie, Forsyth, New Hanover, Stokes and Yadkin counties, NC | $0, 1 per year | $250, or $350 at a PLUS provider |
| Humana Gold Plus Giveback H1036-305 (HMO), Broward, Miami-Dade and Palm Beach, FL | $0, 1 per year | Up to $200, or 2 pairs of select eyeglasses at no cost |
| Humana USAA Honor Giveback H5216-236 (PPO), Alabama | $0 in network | $150, or $250 at a PLUS provider ($150 out of network) |
| Humana USAA Honor Giveback H1036-290 (HMO), Central and North Florida | $0 | $200, or 2 pairs of select eyeglasses at no cost |
2026 vs 2027: Humana Gold Plus H1036-335
This North Carolina plan shows how allowances can change from year to year. Its eyewear allowance rises from $150 (or $250 at a PLUS provider) in 2026 to $250 (or $350 at a PLUS provider) in 2027, while the $0 routine exam stays the same. (2026 Summary of Benefits) The Florida Giveback plan H1036-305 keeps the same vision benefits in both years. If you’re already a Humana member, your Annual Notice of Change lists any 2027 change to your plan’s vision benefit.
Who accepts Humana vision near me?
Humana’s Medicare Advantage vision network is called the Humana Medicare Insight Network. To find an in-network eye doctor:
- Go to Humana’s provider finder at Humana.com/FindCare, or open Humana’s find an eye doctor page and choose “My vision coverage is included with my Medicare Advantage plan.”
- Enter your ZIP code, select your plan’s network and search for an optometrist or ophthalmologist.
- Look for the PLUS indicator if your plan pays a higher allowance at PLUS providers.
- Call the office, confirm it takes your specific Humana plan, and say you’re part of the Humana Medicare Insight Network.
Humana’s 2027 Summaries of Benefits note that the network for Medicare-covered eye care, such as diabetic eye exams or cataract follow-up, can differ from the supplemental routine vision network, so check both if you need medical eye care. Retail optical chains may also participate; for one example, see does America’s Best take Medicare.
What Original Medicare covers for vision
Every Humana Medicare Advantage plan must cover at least what Original Medicare covers. Original Medicare doesn’t pay for routine eye exams for glasses or contacts, but Part B does cover:
- Diabetic eye exams: once a year to check for diabetic retinopathy if you have diabetes. (Medicare.gov)
- Glaucoma tests: once every 12 months if you’re at high risk, for example if you have diabetes or a family history of glaucoma, are African American and 50 or older, or are Hispanic and 65 or older. (Medicare.gov)
- Glasses after cataract surgery: one pair of eyeglasses with standard frames or one set of contact lenses after surgery that implants an intraocular lens. (Medicare.gov)
In Humana’s 2027 plans these Medicare-covered services have their own copays: for example, H1036-335 lists a $0 diabetic eye exam, $0 eyewear after cataract surgery and a $5 copay for other Medicare-covered vision services. Medicare Supplement (Medigap) policies don’t add routine vision. For the full rules, see does Medicare cover eye exams.
Humana Vision PLUS: stand-alone vision insurance
Humana Vision PLUS is a separate individual vision insurance plan sold on Humana.com, not part of Medicare. Humana lists these benefits:
- Eye exam: $0 at a PLUS provider, $10 at other in-network providers, $30 allowance out of network
- Frames: $250 allowance at a PLUS provider, $200 at other in-network providers
- No waiting periods
Humana’s policy documents name EyeMed as the third-party vendor for Vision PLUS. Premiums depend on your state and age, benefits vary by state, and the plan isn’t sold in every state. (Humana: Vision PLUS) Humana also offers Humana Extend plans that bundle dental, vision and hearing. If you’re on Medicare, compare a stand-alone policy with the vision benefit already included in a Medicare Advantage plan before paying a separate premium.
How to choose a Humana plan for vision in 2027
- Find the vision row in each plan’s 2027 Summary of Benefits: the exam copay, the eyewear allowance and whether PLUS providers pay more.
- Check your eye doctor in the Humana Medicare Insight Network.
- Think about what you buy. If you need new glasses every year, a $350 allowance is worth more than a $150 one; if you rarely do, focus on other costs.
- Compare the bigger costs first: doctors, drugs, premium and the out-of-pocket maximum. Our plan comparison worksheet helps.
- Enroll by December 7, 2026 for coverage starting January 1, 2027.
